Take S6 from Hauptbahnhof München in the direction to Tutzing to Gräfelfing (it goes every 20 minutes).
Take bus 268 from Gräfelfing station in the direction to Klinikum Großhadern, get out at Gräfelfing, Neuer Friedhof.

Or take U6 from Sendlinger Tor in the direction to Klinikum Großhadern, then take bus 268 from Großhadern station in the direction to Planegg, Bräuhausstraße. Get out at Gräfelfing, Neuer Friedhof.

Lochhamer Schlag 12 is right at the corner.
